The Pacific Health Resilience program partners with ministries in American Samoa and Hawaii to address a chronic disease burden that the existing system was never designed to carry. The program is anchored on the Rural Health Transformation award and on a working partnership with Tripler Army Medical Center.

Background

American Samoa has one of the highest rates of obesity, diabetes, and cardiovascular disease in the world. Hawaii carries a Rural Health Transformation responsibility that crosses Oahu, the Big Island, Maui, and Molokai. Existing infrastructure was designed for episodic care, not for the longitudinal management these populations need.

What Hydor proposes

The proposal is scoped and concrete. A Sovereign Health Node serves the territory. HealthID provides the longitudinal record across providers and across the diaspora. Dr. Kai is the clinical surface for chronic care, maternal health, and behavioral support. HealthDx scoring identifies populations that benefit from earlier intervention.

Field interpreter

Pacific clinics serve patients across Samoan, English, and other Pacific languages. Dr. Kai on a clinician's wearable carries the conversation in real time. The Conversational Intelligence layer keeps a longitudinal memory of how each patient prefers to be spoken to, so the second visit picks up where the first one ended.

Partners

Named partners include Tripler Army Medical Center and the relevant territorial and state ministry counterparts. Additional partners are added as the program scopes phase one deployment.

Status

The program is in the design phase. The Office of the CIO has the live briefing package. A sovereign briefing is the entry path for additional ministry or federal counterparts who want to be part of phase one.
